Hospice Chaplain










Responsibilities: The Hospice Chaplain is an integral part of the hospice interdisciplinary team and is responsible for assessing the spiritual needs of the hospice patient and family. The Chaplain must be prepared to care for patients of diverse cultures and/ or religious backgrounds, understanding that spiritual services must be delivered in a manner that is consistent with patient/family beliefs and desire for service.   The Chaplain coordinates spiritual services offered through the hospice staff, local religious organizations or community clergy. Provides funeral services when requested, and assists family with this planning. Provides bereavement support.   Assists with public speaking/training concerning spiritual issues, end of life concerns and hospice care for other health care providers.

Social Worker








Responsibilities: As an integral part of the Hospice interdisciplinary team, the Social Worker is responsible for the development and oversight of a plan of care for an assigned group of patients/families’ psychosocial, financial, and emotional needs.     Liaisons with community resources to find support for the patient/families as needed.  Flexibility with schedule.   Assists with mentoring of interns and other students.  Assists with facilitating bereavement programs as needed.    Delivers care under the guidance of the best standard of practice, Circle of Life policy and procedures and the Arkansas Social Work licensure board.

Bereavement Counselor














Responsibilities Provides grief counseling services to help meet patient/family social and emotional needs in relation to the end of life issues, loss and grief. Provides on-site and follow-up (both in person and via telephone) clinical interventions/consultations. Provides grief counseling services for new, continuing and crisis patients: for individuals, couples, families. Provides critical incident, stress management and bereavement services. Conducts initial assessments, maintains updates care plan as needed. Documents all contacts made with outside agencies and health care professionals. Makes use of community resources/referrals. Consults clinical supervision on an as-needed basis. Use of Electronic Medical Record system on a daily basis.  Collaborate in program development, Supports staff and Long Term Care facilities with bereavement issues as needed.  Accountable for direct delivery of bereavement care services provided by Circle of Life Hospice.  Assess, plan, implement and document appropriate interventions.
