The Care Navigation Program at Circle of Life is a free, non-medical, service designed for individuals who are facing a serious, life-limiting illness. It supports you or your loved one through the mental, physical, and emotional changes that may accompany a life-limiting illness.
Our Care Navigation Program offers suggestions to enhance the quality of your life or that of your loved one. It is up to the client to choose what would be of help to them. It can help you or your loved one:
- Understand their illness and offer emotional support to ease feelings of isolation
- Navigate and connect with available community resources
- Arrange for respite care, socialization and more
- Link up with a variety of non-medical assistance through a network of trained volunteers
We have found that individuals and families in our service area benefit most from the Care Navigation Program when their disease poses daily life challenges sufficient to create hardship, but not sufficient enough to bring them onto hospice.
The Care Navigation services are offered at no charge to our clients. They are provided by staff and a network of trained volunteers under the supervision of our Care Navigation Coordinator. Additional services are provided through a variety of community organizations and agencies.
Accessing program services
While a physician’s referral is always welcome, it is not necessary to access Care Navigation services. You or a family member can call directly and ask about your service options.
To access Circle of Life’s Care Navigation Program, please call 479-750-6632, Monday-Friday 8 a.m. – 4:30 p.m. A team member will perform an initial assessment and a service plan will be developed. A referral to other resources may be offered at any time if a higher level of care is needed.
