RN Admission Specialist
Primarily responsible for coordination and execution of the admission process to hospice care. May carry a small case management load along with coverage for patient visits. Effectively communicate the hospice concept to families and patient. Develop the plan of care and effectively communicate the patient/family care needs to the team, establishing an appropriate frequency of disciplines to meet the patient/families’ needs. Required to be flexible and change schedule at a moment’s notice to meet patient needs. Required to travel daily and manage own schedule effectively. Must have excellent clinical skills and be able to identify problems, consult with the physician, and execute orders to meet patient’s needs.
Requirements: Arkansas RN license without restriction. Minimum 5 years hospice, home health or acute care nursing experience. 3 years related management experience. Hospice knowledge preferred. Requires ability to coordinate patient care needs with families and team members; effectively and efficiently coordinate the care of a group of patients in a timely manner. Ability to stay calm under stressful conditions. Requires excellent problem solving, clinical and critical thinking skills. Ability to make changes and flexibility with schedule and time to meet the needs of the patients and organization. Ability to coordinate ancillary staff, DME, and Pharmacy needs for a group of patients. Communicate effectively with others verbally, in writing, and via phone and e-mail. Must be able to deal with human suffering while maintaining professional boundaries. Computer skills and excellent documentation skills required. May require weekend shifts. Ability to be on-call as needed.
Education: An Associate degree or Bachelor’s of Science degree in Nursing.
