RN Case Manager, Full Time
Reports to Outpatient Clinical Care Coordinator
Responsibilities: Coordinates total care for case load of hospice patients and their family in the field. Provides oversight to hospice aides on assigned group of patients. May be required to fulfill RN On Call duties as needed.
Requirements: Current unrestricted Arkansas RN license. Requires current BLS certification, satisfactory results of current TB skin test and annual flu vaccination. Strong nursing background in a hospital or healthcare facility; prefer national hospice & palliative care certification and five years of hospice, home health or acute care nursing experience. Excellent clinical and critical thinking skills and high level of problem solving, decision making skills and ability to work with numerous family issues. Requires ability to stay calm & cope with human suffering and emergencies. Strong documentation and communication skills. Computer skills required. Experience using electronic health records system preferred.
Position requires use of personal vehicle, a valid driver’s license and current auto insurance as required by law. Requires long hours walking or standing. May require long work days to ensure patient coverage. Frequently exposed to unsanitary home conditions, driving in isolated areas or heavy traffic. Must be able to bend, lift, stand and sit for extended periods of time and safely lift and carry 50 pounds.
Education: Graduate of an accredited school of nursing; an associate degree or Bachelor’s of Science degree in Nursing.
