RN Admission/Intake Specialist

FT, 30+ hrs/wk

Responsibilities:       Primarily responsible for coordination and execution of the admission process.   Develops and manages initial contact as necessary with discharge planner, family, or responsible party to initiate the hospice referral.  Collection of data in regard to the referral process.  Assists with the admission process.  Facilitates the timely conversion of referrals to admissions through coordination with team members and other departments.  May carry a small case management load along with coverage for patient visits.  Effectively communicates the Hospice concept to families and patient.   Effectively communicates the patient/family care needs to the team, establishing an appropriate frequency of disciplines to meet the patient/families’ needs.


Requirements:   Current Arkansas RN license without restriction, current BLS certification.  Hospice knowledge and hospice, home health or acute care nursing experience preferred, with related management experience.     Excellent critical thinking and clinical skills.  Ability to effectively and efficiently coordinate the care of a group of patients in a timely manner.   Ability to deal with human suffering while maintaining professional boundaries.   Ability to stay calm under stressful conditions.


Ability to work closely and communicate effectively with others verbally, in writing, and via phone and e-mail.   Excellent documentation skills.   Computer skills required.   Requires flexibility with schedule and time to meet the needs of the patients and organization.     Requires evening and weekend availability and participation in routine on-call rotation.  Required to travel daily and manage own schedule effectively.  Use of personal vehicle, current driver’s license and auto insurance as required by law.  May require long work days to ensure patient coverage.   Must be able to bend, lift, stand and sit for extended periods of time and safely lift and carry 50 pounds.


Education:   Graduate of an accredited school of nursing; an associate degree or BSN.  Hospice certification preferred.
