Care Navigation
Care Navigation at Circle of Life is a free, non-medical, service designed for individuals who are facing a serious, life-limiting illness. It supports you or your loved one through the mental, physical, and emotional changes that may accompany a life-limiting illness.
Care Navigation addresses the needs of patients in a declining state of health who are not yet ready to enter hospice care. When you have a serious illness, you may be navigating an unfamiliar world of treatment options, doctor’s visits and difficult conversations.
We offer a Care Navigator to partner with you to support your needs, offer assistance with valuable community resources and walk with you along your healthcare journey.
Who is eligible? Anyone facing a serious, life-limiting illness and reside in Benton, Washington, Carroll or Madison county.
Cost for service? Care Navigation is provided as a no cost, non-medical service to the community and is funded through community donations.
Care Navigator support A Care Navigator can provide emotional support for the patient and family, help monitor changing social and support needs and link you to community resources.
Nurse Navigator support A Nurse Navigator is available for non-medical assessments and informational visits.

Care Navigation works with you to develop a plan best suited to assist you along your healthcare journey. Our support services are available primarily via phone conversations and follow-ups at a frequency that suites each individual. Face to face visits available if needed.
Accessing program services
While a physician’s referral is always welcome, it is not necessary to access Care Navigation services. You or a family member can call directly and ask about your service options.

A team member will perform an initial assessment and a service plan will be developed. A referral to other resources may be offered at any time if a higher level of care is needed.
